As part of our Transitional Support portfolio, Bellwoods is seeking (1) part-time Physiotherapist (22.5 to 30 hours per week) to join our team on contract. The successful candidate will join an already established multidisciplinary team to offer enhanced support to people transitioning from hospital to community, promoting and supporting their safety and success.

The Role:

This is an exciting opportunity for a Physiotherapist (PT) who enjoys working in a fast paced, team-oriented environment focused on creative problem solving. Based out of our 300 Shaw Street supportive housing site and reporting directly to the Director, Transition and Special Projects, the successful PT will be responsible for triaging applicants, performing comprehensive in-home assessments and delivering treatment services to achieve client/caregiver goals as specified during collaborative service planning. The PT will assume an integral role within the dynamic, multi-service team which assists individuals to move from hospital to various community-based destinations. In this role, the PT will also be required to work with a variety of community-based clients towards positive health outcomes enabling their continued independence. He/she will also provide direct and indirect support to staff, including teaching, as they implement interventions aimed at maintaining or improving the health, safety and well-being of clients in their chosen environment. This position involves:

  • Conducting comprehensive assessments and developing goal-oriented treatment plans for clients experiencing acute and/or complex chronic conditions; prescribing and implementing equipment/assistive devices; as well as providing education/teaching for support personnel/caregivers with the goal of optimizing time at home/reducing re-admit
  • Providing assessment and treatment services to adults and older adults who experience physical functional challenges, but who may also have challenges related to homelessness, mental health and/or addiction(s), cognition challenges and/or responsive behaviours
  • Participation in the planning, implementation and evaluation of team-based service plans in partnership with clients, their families, as well as a small, but intensive, multi-disciplinary team made up of members from Bellwoods and our partner organizations
  • Collaborating with clients’ support network to achieve identified objectives and health outcomes which facilitate clients’ progress toward stated service plan goal(s)
  • Performing administrative duties such as preparation of reports, workload measurement, providing data as requested to inform program activities, etc.
  • Participation in program development, as needed, to define program practices, measurement indicators, and documentation/collection tools
  • Applying a continuous quality improvement approach to practice to continually iterate and improve program outcomes

Requirements:

  • Baccalaureate/Masters degree in Physical or Physio Therapy
  • A member in good standing with the College of Physiotherapists of Ontario (CPO)
  • Two (2) to five (5) years recent experience in community setting, strongly preferred
  • Experience and knowledge relevant to Physiotherapy best practices and evidence-based initiatives as it relates to providing services to a population of clients that includes complex, fragile seniors and supports their ability to successfully age in place
  • ADP Mobility Device Authorizer is a strongly preferred
  • Strong assessment, negotiation and clinical decision-making skills. Experience with interRAI CHA is considered an asset.
  • Working knowledge of community programs and services, and the initiative to proactively search for new resources including additional supports and services to meet client/caregiver goals
  • Knowledge of Ontario Health System legislation and best practice (Public Hospitals Act, Substitute Decision-Making Act, Consent to Treatment Act, Health Care Consent Act, Mental Health Act, etc.) and applies this knowledge to ensure positive client outcomes
  • Ability to effectively prioritize, apply critical thinking and problem solving to mitigate risks and navigate challenging or complex conversations/situations to achieve a positive resolution
  • Demonstrated ability to be flexible in response to shifting daily caseload demands, be responsive to the need for urgent response and driven to provide a positive client and partner experience throughout service provision
  • Significant travel within the Toronto Central LHIN boundaries will be required.
