Who is Eligible?
You are eligible for Bellwoods’ programs and services if you:
- You may be eligible for Bellwoods programs and services if you:
- Are 16 years of age or older
- Have a physical disability
- Need help with daily activities to live in the community
- Live in the Toronto area
- Are able, or willing to learn, to direct your own support services
- Have a valid Ontario Health Card (OHIP)
How To Apply For Services
Most Bellwoods services, including Attendant Outreach Services and Supportive Housing, require an application through the Attendant Service Application Centre (ASAC).
ASAC is part of the Centre for Independent Living in Toronto (CILT) and manages applications for multiple service providers across Toronto.

- Services are offered when availability matches your needs
Referral Programs
Housing with Supports
This program is referral-based. Referrals are typically made by hospitals, transitional programs, or community partners.
Transition Programs (Reintegration Care Units)
Referrals are made through hospital partners.
