Multi-Sector Service Accountability Agreement
The Local Health System Integration Act, 2006 requires that the Local Health Integration Network (LHIN) and the Health Service Provider (HSP) enter into a service accountability agreement. The service accountability agreement supports a collaborative relationship between the LHIN and the HSP to improve the health of Ontarians through better access to high quality health services, to co-ordinate health care in local health systems and to manage the health system at the local level effectively and efficiently.
Bellwoods Centres for Community Living Inc. (Bellwoods) and the LHIN agree that the LHIN will provide funding to Bellwoods on the terms and conditions set out in this M-SSA Agreement to enable the provision of services to the local health system by Bellwoods.

Compliance to Privacy Legislation
Bellwoods is a health information custodian (HIC) under the Personal Health Information Protection Act (PHIPA), 2004. This provincial legislation establishes rules concerning the collection, use and disclosure of personal health information. As a health information custodian, Bellwoods and Bellwoods’ staff are responsible for ensuring that the personal health information of all applicants and clients is treated with respect and sensitivity.
Under PHIPA, Bellwoods complies with the Ten Privacy Principles, as described below:

Personal Information of Clients
An information sheet on Privacy is provided to all Bellwoods clients to ensure that they are aware of the collection, appropriate use and sharing of personal health information practice and procedures.
