The Role:

This is an exciting opportunity for health care professional who enjoys working in a fast paced, team-oriented environment that requires creative problem solving and a dedication and focus on client success in independent community living.  The successful candidate will join an already established multidisciplinary team focused on transitioning clients from hospital and community to the reintegration care unit (RCU) and/or permanent supportive housing (SH) at Bellwoods as well as keeping our existing Bellwoods community clients well supported and resourced to optimize their safety and community living experience.  Based out of our 300 Shaw Street Community Connect re-integration care unit and supportive housing (SH) site, and reporting directly to the Director, Transition and Community Partnerships.  The Independent Living Facilitator (ILF) will be working directly with the clients and their personal support network as well as collaborating and partnering with multiple Teams, including Bellwoods client support team staff, hospital partners, community health agencies, other RCUs, vendors, and other community agencies. To achieve these, the position involves:

  • Embodying anti-racist, anti-oppressive, equity-seeking practices in all aspects of work, including with participants, community members, volunteers, neighbours, Bellwoods staff, stakeholders, networks, and others
  • Completion of comprehensive intake and eligibility assessments with adults and older adults with physical disabilities for consideration in our Community Connect re-integration unit or permanent supportive housing units. Includes assessment of physical, cognitive and psychosocial/psycho-emotional functioning for individuals who have a physical disability, but who may also have challenges related to homelessness, mental health and addiction, or other social determinants of health.
  • Analysis of assessment findings to allow for comprehensive, client-centred co-designing and establishing of a holistic plan to optimize community living independence, safety and wellbeing. This will include consideration of the client’s goals, wishes and needs as well as best practice, and resource availability.
  • Lead the transition planning for clients into and out Bellwoods programs, supportive housing or transitional units with the goal of creating seamless transitions. For example, the Independent Living Facilitator will lead the transition planning for Bellwoods, assisting clients, families and hospitals in creating a plan for safe, efficient and positive moves to community from hospital (or other community settings) for those clients entering Bellwoods transitional housing program (reintegration care unit [RCU]).
  • Provide ongoing coaching, navigation and support to clients planning their next transition in community by completing team based, holistic transition plans centered on client directed goals and objectives.
  • For existing Bellwoods client in our SH or our Attendant Outreach (AO) Program, the ILF provides ongoing support to the client and their support team to develop and execute a comprehensive, goal-oriented plan to optimize their independence, safety and wellbeing in the new community setting through: equipment recommendations; home modification recommendations; education; recommendations supportive care provision (by PSW and/or personal caregivers); and/or navigation and linkage to other community resources (housing, financial support).
  • Provision of direct and indirect support to Bellwoods staff as they implement service directives for best practices
  • Performing administrative duties such as preparation of reports, committee participation and providing input into policy and procedure development.
  • Participation in program development including updating and evaluating of tools (forms, computer programs) that facilitate the work, as appropriate
  • Presentation of educational workshops to community
  • Experience with delegation to support personnel to carry out goal directed teaching, facilitation or case management activities.

Requirements:

  • Degree/diploma or equivalent in Occupational Therapy, Physiotherapy, or Nursing
  • A member in good standing with applicable regulatory body
  • Two (2) to five (5) years recent experience in community setting, preferred but new graduates with relevant community experience are encouraged to apply
  • Bellwoods is a service provider covered under the Home Care and Community Services Act.
  • Experience and knowledge relevant to professional best practices and evidence-based initiatives as it relates to independent living
  • Strong clinical assessment and treatment skills and case management/care coordination skills, required
  • ADP Mobility Device Authorizer, an asset
  • Knowledge of Mental health and Addiction issues, an asset.
  • Strong assessment skills. Experience with interRAI CHA, an asset.
  • Working knowledge of community programs and services, and the experience and/or initiative to proactively search for new resources including housing and services
  • Hospital to community transition planning experience, an asset
  • Knowledge of Ministry of Health operations and regulations (Public Hospitals Act, Substitute Decision-Making Act, Consent to Treatment Act, Health Care Consent Act, Mental Health Act, etc.)
  • Excellent communication skills verbal and written, in particular, the ability to negotiate, mediate and advocate.
  • Demonstrated ability to work with individuals, their care team members, caregivers and others in their support network to achieve their goals and support them to remain in a community-based environment.
  • Ability to work independently as well as within a multidisciplinary team, multitask and set priorities
  • Ability to travel to multiple work/assessment sites is essential
  • Experience and demonstrated skill overseeing delegated treatment plans to support personnel.
  • Legally entitled to work in Canada.

Primary Responsibilities:

  • Perform client needs assessments and reassessments (through the use of tools such as InterRAI CHA and Alayacare), ongoing case management, and care coordination.
  • Work with potential clients to sign Service Agreements and where applicable Tenancy Agreements
  • Work with clients to co-design care plans based on assessed needs
  • Provide ongoing clinical support, supervision, orientation, education and training to personal support workers.
  • Contribute to the delivery of support services through team building, excellent customer service, continuous quality improvement and risk management
  • Support quality care and client/worker safety by managing a caseload of complex needs clients, participating in group supervision meetings and providing clinical consultation with the team as required
  • Facilitate and support scheduling, payroll submissions and audits, performance management and quality monitoring of support services consistent with related service agreements and governing collective agreements while ensuring the health and security of clients and staff
  • Liaise with external providers, community providers, and property managers for client services.

Requirements:

  • Post-secondary education in either social sciences or a Degree in Nursing or Diploma/Degree in Practical Nursing or related field, combined with relevant work experience responding to diverse client services and/or relations challenges in a supervisor role;
  • 3-5 years PSW supervisory and management experience, preferably in a related community support service unionized environment;
  • Previous experience supporting the activities of daily living for adults with disabilities, seniors and/or clients with mental health challenges;
  • Ability to balance many competing priorities at one time;
  • Outstanding clinical assessment skills with good judgement in understanding clients’ needs and utilizing assessment tools (interRAI (CHA) system preferred or Alayacare);
  • Previous scheduling experience of PSW staff within a related community support service environment, preferably including supportive housing;
  • Knowledge and understanding of related legislation including the Occupational Health & Safety Act, Regulated Health Professions Act and activities of daily living provisions;
  • Excellent interpersonal skills and communication, both written and verbal are necessary for effective interaction with all levels of staff, clients and other members of the public;
  • Demonstrated competency with MS Office and related software applications is essential;
  • Previous use or experience with data management systems such as MS SharePoint and Alayacare is an asset.
  • Able to travel to various locations throughout the week.

Personal Support Worker (PSW)

Starting Rate: $23.67 (which includes the provincial government’s $3 PSW Wage Enhancement)

Job Summary: Bellwoods provides pre-booked personal support services for individuals with physical disabilities living in the community. These services can be provided in the client’s home, place of employment, and/or educational institution, where clients are pursuing a degree, diploma or certificate. Bellwoods provides personal support services to people 16 years of age and older, living in their homes, who have a permanent physical disability and require assistance with their activities of daily living. Services are available throughout the Greater Toronto Area, between the hours of 6:00 am and 12:00 midnight for up to 3 hours per day in the community and 24/7 in our supportive housing locations, and on a pre-scheduled visitation basis.

Location: GTA

Duties/Responsibilities: Our community support services are client-directed and our Support Service Worker’s assist clients to carry out their activities of daily living as outlined in an individual support service agreement and individual service plan. Our SSWs provide services such as:

  • personal grooming and hygiene
  • rising and retiring
  • positioning and transferring
  • dressing and undressing
  • eating
  • toileting
  • light meal preparation

 

Skills/Qualifications :

  • Completed Personal Support Worker certificate required.
  • Eligible candidates must be able to work non-traditional hours including early mornings, evenings, weekends and holidays.
  • Related work experience, within the community, with adults with physical disabilities an asset
  • Good oral and written communication skills are essential

Services are provided to adults (16+ years of age) with physical support needs, seniors, and individuals with mental health and addiction challenges as well as those at risk of homelessness.
