Primary Responsibilities:

  • Perform client needs assessments and reassessments (through the use of tools such as InterRAI CHA and Alayacare), ongoing case management, and care coordination.

  • Work with clients to co-design care plans based on assessed needs

  • Post-secondary education in either social sciences or a Degree in Nursing or Diploma/Degree in Practical Nursing or related field, combined with relevant work experience responding to diverse client services and/or relations challenges in a supervisor role.
  • 3-5 years PSW supervisory and management experience, preferably in a related community support service unionized environment;
  • Previous experience supporting the activities of daily living for adults with disabilities, seniors and/or clients with mental health challenges;

The Role:
This is an exciting opportunity for health care professional who enjoys working in a fast paced, team-oriented environment that requires creative problem solving and a dedication and focus on client success in independent community living. The successful candidate will join an already established multidisciplinary team focused on transitioning clients from hospital and community to the reintegration care unit (RCU) and/or permanent supportive housing (SH) at Bellwoods as well as keeping our existing Bellwoods community clients well supported and resourced to optimize their safety and community living experience. Based out of our 300 Shaw Street Community Connect re-integration care unit and supportive housing (SH) site, and reporting directly to the Director, Transition and Community Partnerships. The Independent Living Facilitator (ILF) will be working directly with the clients and their personal support network as well as collaborating and partnering with multiple Teams, including Bellwoods client support team staff, hospital partners, community health agencies, other RCUs, vendors, and other community agencies. To achieve these, the position involves:

  • Embodying anti-racist, anti-oppressive, equity-seeking practices in all aspects of work, including with participants, community members, volunteers, neighbours, Bellwoods staff, stakeholders, networks, and others
  • Completion of comprehensive intake and eligibility assessments with adults and older adults with physical disabilities for consideration in our Community Connect re-integration unit or permanent supportive housing units. Includes assessment of physical, cognitive and psychosocial/psycho-emotional functioning for individuals who have a physical disability, but who may also have challenges related to homelessness, mental health and addiction, or other social determinants of health.
  • Analysis of assessment findings to allow for comprehensive, client-centred co-designing and establishing of a holistic plan to optimize community living independence, safety and wellbeing. This will include consideration of the client’s goals, wishes and needs as well as best practice, and resource availability.
  • Lead the transition planning for clients into and out Bellwoods programs, supportive housing or transitional units with the goal of creating seamless transitions. For example, the Independent Living Facilitator will lead the transition planning for Bellwoods, assisting clients, families and hospitals in creating a plan for safe, efficient and positive moves to community from hospital (or other community settings) for those clients entering Bellwoods transitional housing program (reintegration care unit [RCU]).
  • Provide ongoing coaching, navigation and support to clients planning their next transition in community by completing team based, holistic transition plans centered on client directed goals and objectives.
  • For existing Bellwoods client in our SH or our Attendant Outreach (AO) Program, the ILF provides ongoing support to the client and their support team to develop and execute a comprehensive, goal-oriented plan to optimize their independence, safety and wellbeing in the new community setting through: equipment recommendations; home modification recommendations; education; recommendations supportive care provision (by PSW and/or personal caregivers); and/or navigation and linkage to other community resources (housing, financial support).
  • Provision of direct and indirect support to Bellwoods staff as they implement service directives for best practices
  • Performing administrative duties such as preparation of reports, committee participation and providing input into policy and procedure development.
  • Participation in program development including updating and evaluating of tools (forms, computer programs) that facilitate the work, as appropriate
  • Presentation of educational workshops to community
  • Experience with delegation to support personnel to carry out goal directed teaching, facilitation or case management activities.

Requirements:

  • Degree/diploma or equivalent in Occupational Therapy, Physiotherapy, or Nursing
  • A member in good standing with applicable regulatory body
  • Two (2) to five (5) years recent experience in community setting, preferred but new graduates with relevant community experience are encouraged to apply

The Housing Access Worker will be responsible for facilitating the finding and securing of appropriate housing opportunities for HLS program participants.

The Housing Access Worker will be joining an already established multidisciplinary team focused on transitioning clients from homelessness, hospital/reintegration care units with no discharge destination or precarious/temporary housing to permanent, sustainable housing options, while also providing them with the tools to maintain that housing (budgeting, relationship building) and work on stabilizing health, improving social connections and helping them focus on other goals such as volunteerism, education and/or employment.

Responsibilities:
•Working with the successful program candidates and their support network on developing a client specific action plan for searching for and securing sustainable housing in the city of Toronto and surrounding area.
•Under the direction of the Community Transitions Coordinator, carrying out activities directly with clients to support the achievement of their housing goals. This includes completing referrals and connect clients to supports that that empower them to move beyond vulnerability and marginalization into sustainable, long-term living situations that support the client’s ability to be self-sufficient in the community.

The Role:

IT Compliance, Budget, Operations, Procurement, and Service Management
• Ensure compliance with internal and external regulations regarding IT/IM procurement, security/privacy, and support organizational compliance as applicable outside of IT/IM
• Work with the finance team to develop and manage an annual IT budget that adheres to ministerial guidelines for the organization
• Develop and implement strategies to improve the function and value of IT at Bellwoods

The Community Transitions Coordinator will be responsible completing all intake for clients seeking support in finding permanent housing through the Housing with Layered Supports (HLS) program, will support the Housing Access Worker seeking housing for the client and then will focus on how to stabilize the client in housing and create a client-centred, wrap-around, sustainable community independent living plan with strong community partnerships.

The Community Transitions Coordinator will work with an established multidisciplinary team focused on transitioning clients to permanent housing solutions from hospital and reintegration care units, who have no or precarious discharge destinations and limited housing options due to mobility or physical challenges related to injury or chronic illness or disease. The Team is focused on supporting clients move to permanent, sustainable housing options, while also providing them with the tools to maintain that housing (budgeting, relationship building, system navigation) and work on stabilizing health, improving social connections and helping them focus on other goals such as volunteerism, education and/or employment.

Responsibilities:

•Leading all intake assessments for candidates for the Housing with Layered Supports (HLS) Program and consulting in Bellwoods Client selection for re-integration care unit (RCU) to ensure the client’s identified housing goals can be met within the time parameters of these programs and the client’s financial resources and personal skills/experience and/or needs.

•In collaboration with the Housing Access Worker, providing navigation and support that would directly support the achievement of the clients housing search and acquisition goals, coaching and supporting the client to move beyond vulnerability and marginalization into sustainable, long-term living situations that support the client’s ability to be self-sufficient and safe in the community.

•Support and provide oversight and direction to the client and Housing Access Worker in move-in planning and optimization of access to all resources to facilitate a safe and effective move in to their new housing.

•Working with the client on setting their own SMART goals around maintaining a home, landlord relationship building, financial stability/budgeting, improved health, community engagement, and other goals such as volunteerism, education or employment, using biopsychosocial model, motivational interviewing, client supportive counselling, direct education and coaching.

•Ongoing coaching, navigation, linking, education, case management and care coordination will follow to ensure the client is connected to resources and community partners to support them and that they develop the tools, resilience and knowledge to maintain housing and to achieve their goals.

•Funds will be available for the Community Transitions Coordinator to use responsibly to remove barriers and challenges faced by clients in transition to community and in goal achievement for long-term sustainable housing stability. Clear accounting and reporting will be required.

•Develop a strong working relationship and evolve a network of community partners and collaborators to support the client in their goal achievement. These may include: other community support programs; family health care teams; supportive housing providers; addiction and harm reduction programs; social program providers; volunteer programs; food security resources; landlords; tax experts; Public Guardian and Trustee staff; advocacy groups; cultural specific support programs; the funder; experts in Housing First ideology; equipment vendors; home and community support service providers; and many more. A clients unique goals may dictate a unique partnership.

•Develop a strong working relationship and evolving a network/resource list with housing community partners including: housing support agencies; landlords; boarding home providers; support agencies; supportive housing programs; movers; ODSP; Furniture Bank etc.

•Strong advocacy role on behalf of clients with landlords, service providers, and other community members

•Clear and accurate documentation of all activities in the client chart and maintaining TGRIP data entry and HLS client tracking tools for necessary quarterly reporting to the funder and senior leadership.
