Reintegration Care Units (RCU) support hospital discharge for individuals with physical disabilities who need short-term housing and personal support before returning to the community.
The program provides accessible apartments and support services to help people rebuild daily living skills and prepare for independent community living.
Who This Program Is For
This program is designed for adults who:
- Are currently in hospital and preparing for discharge
- Have experienced a significant illness, injury, or change in physical function
- Require an accessible environment and personal support
- Need time to rebuild skills and confidence for independent living
How the Program Works
Participants live in one of 15 fully accessible apartments at Bellwoods’ Shaw Street site.
During their stay, clients receive support from a team that helps them:
- Set goals for community living
- Rebuild daily living skills
- Learn how to direct their own support services
- Access equipment and community resources
- Plan for housing and supports after the program
Support services may include help with:
- Personal care and hygiene
- Mobility and transfers
- Meal preparation
- Daily routines and household tasks
All services are non-medical and focus on building independence.
Length of Stay
Participants may stay in the program for up to four months, depending on their goals and transition plan.

How to Access the Program
Referrals to the Reintegration Care Units program are made through hospital partners.
Hospitals submit referrals through the Resources Matching and Referral (RM&R) system as part of discharge planning.
