Reintegration Care Units

Reintegration Care Units (RCU) support hospital discharge for individuals with physical disabilities who need short-term housing and personal support before returning to the community.

The program provides accessible apartments and support services to help people rebuild daily living skills and prepare for independent community living.



During their stay, clients receive support from a team that helps them:

  • Set goals for community living
  • Rebuild daily living skills
  • Learn how to direct their own support services
  • Access equipment and community resources
  • Plan for housing and supports after the program

Support services may include help with:

  • Personal care and hygiene
  • Mobility and transfers
  • Meal preparation
  • Daily routines and household tasks

Participants may stay in the program for up to four months, depending on their goals and transition plan.


Reintegration Care Units consists of 15 barrier-free apartments located at:

300 Shaw Street
Toronto, Ontario


Referrals to the Reintegration Care Units program are made through hospital partners.

Hospitals submit referrals through the Resources Matching and Referral (RM&R) system as part of discharge planning.
