Reintegration Care Units

Bellwoods Reintegration Care Units (RCUs) provide patients who are ready to be discharged from hospital with a short-term, safe and supportive, place to transition to in the community. Working in partnership with other community providers and hospitals, RCUs often prevent unnecessary admission to a long-term care home. Bellwoods operates 15 wheelchair accessible reintegration care units.

RCUs focus on an individual’s goals and needs for re-activation and allow time and support to plan and prepare for transition to a more permanent setting in the community (e.g., home, new community housing, supportive housing, other assisted living environment). Clients accepted into a RCU need to have a post-RCU transition destination or plan to work toward confirming a plan at the time of referral.

Bellwoods co-chairs the Transitional Leadership Committee with Ontario Health (OH) that oversees the community-led RCUs in the Toronto Region. The committee includes membership from all the community-based RCU providers and meets on a quarterly basis to discuss priorities, challenges, opportunities, and monitors outcomes and capacity with regard to the RCU programs.

Referrals for these programs take place through the Resource Matching & Referral (RM&R) platform.
