Reintegration Care Units

Bellwoods Reintegration Care Units (RCUs) provide patients who are ready to be discharged from hospital with a short-term, safe and supportive, place to transition to in the community. Working in partnership with other community providers and hospitals, RCUs often prevent unnecessary admission to a long-term care home. Bellwoods operates 15 wheelchair accessible reintegration care units.

RCUs focus on an individual’s goals and needs for re-activation and allow time and support to plan and prepare for transition to a more permanent setting in the community (e.g., home, new community housing, supportive housing, other assisted living environment). Clients accepted into a RCU need to have a post-RCU transition destination or plan to work toward confirming a plan at the time of referral.

Bellwoods co-chairs the Transitional Leadership Committee with Ontario Health (OH) that oversees the community-led RCUs in the Toronto Region. The committee includes membership from all the community-based RCU providers and meets on a quarterly basis to discuss priorities, challenges, opportunities, and monitors outcomes and capacity with regard to the RCU programs.

UPDATE ON REFERRAL PROCESS: The referral process for The Reintegration Care Units (RCU) and the
Stepping Stones Project [see below] is undergoing a major change. Bellwoods has operated a Centralized Referral Management (CRM) program for these projects since their inception. However, in 2024 referrals for these programs will now take place through the Resource Matching & Referral (RM&R) platform. As of April 15, 2024, all referrals must be initiated and submitted through RM&R (Direct Link). Below are further instructions on how to initiate the process and utilize the platform:

Stepping Stones Project

Stepping Stones Project (TSSP) is located at LOFT’s John Gibson House.  Launched in 2008, this project helps address the challenges of transitioning seniors (55+) with a history of mental health illness back to the community who were medically ready for discharge but no longer had an appropriate or safe discharge destination due to need for affordable and supportive housing.
