Bellwoods Centres for Community Living provides community-based, non-medical support for people with physical disabilities. We help individuals manage daily life, access safe housing, and receive support in the community.
Services are available to people aged 16 and older and are delivered in homes, housing sites, and community settings across Toronto.
Types of Support We Provide
People come to Bellwoods with different needs. Some require ongoing support, while others need help during a period of transition. Depending on individual circumstances, Bellwoods may provide support in the following areas:
Personal Support Services
We provide non-medical personal support to help with daily activities. Support is planned in advance and based on individual needs and routines. Personal support may be provided:
- In a person’s home
- In a Bellwoods-supported housing
- In community settings
Supportive and Accessible Housing
Bellwoods supports access to safe, accessible housing. People live in their own apartments and receive personal support based on their needs.
Housing is offered through Bellwoods sites and through partnerships with other housing providers.
Transition & Reintegration Supports
Bellwoods provides short-term support for people returning to the community after a hospital stay, illness, or injury.
These supports help with planning, daily living skills, and connecting to longer-term services.
Independent Living Support
Independent Living Support supports planning, skill-building, and problem-solving related to community living. These supports help with planning, daily living skills, and connecting to longer-term services.
This service does not include hands-on personal care. It may be offered on its own or alongside other Bellwoods services.

Our Core Services
Bellwoods delivers many of these supports through the following core programs:

Coordinated Service Delivery
Bellwoods works with healthcare providers, community agencies, and system partners to support coordinated service delivery. Collaboration helps ensure continuity and reduces service gaps during transitions or changes in care.
Accreditation: Accredited with Exemplary Standing
Bellwoods has been accredited with Exemplary Standing by Accreditation Canada, the highest level of accreditation available. This marks the third consecutive time Bellwoods has received this distinction.
In our most recent review, Bellwoods met 100% of all required standards, including system-wide and service excellence standards. This achievement reflects the commitment of our staff and the quality of care and support we provide.
