Independent Living Supports is available exclusively to current Bellwoods clients. This program helps people with physical disabilities build the skills and confidence needed to live safely and independently in the community.
The program provides goal-oriented guidance, education, and problem-solving support. Staff work with clients to identify their needs and connect them to resources, equipment, and services that support independent living.
Independent Living Supports does not provide hands-on personal care services.
Who Is It For?
This program is available to Bellwoods clients who are currently:
✓ Living in Supportive Housing or Reintegration Care Units ✓ Receiving Attendant Outreach services
Clients may contact the program directly or ask their Support Service Supervisor to make a referral.
What Kind of Support Can This Include?
Our staff work with clients to identify goals and provide support in areas such as:
Home safety and accessibility
Mobility, seating, and adaptive equipment
Learning self-care and daily living skills
Household management and routines
Health and wellness planning
Community orientation and navigation
Housing and income supports
Connecting with community services and resources
How Does the Program Work?
Independent Living Supports is delivered by staff who work with clients to identify goals and develop a plan for increasing independence. Support may include:
✓ One-to-one guidance and coaching ✓ Education and skill-building ✓ Coordination with community partners ✓ Recommendations for equipment or services
The program is goal-oriented and time-limited, meaning support continues until the client’s goals have been achieved.
