Housing With
Supports

Housing with Supports helps people with physical disabilities stay housed and live independently in the community. The program provides one-to-one support for people who are moving into housing or have recently been housed after experiencing homelessness or housing instability.

Bellwoods delivers this program in partnership with the City of Toronto.

Who Can Access This Program

You may be eligible if you:

An adult or senior with a physical disability affecting mobility, endurance, or daily living
Are transitioning from hospital care, institutional settings, or the shelter system
Are willing to work with a Case Manager to support independent living
Have a stable income source to cover housing costs
Are not have an active refugee claim (exceptions may be considered case by case)

What Kind of Support Can This Include?

Support is based on your needs and may include help with:

Moving into your home
and getting set up

Building routines to
manage daily life

Connecting with income supports and community services

Accessing furniture, equipment, and basic household items

Navigating healthcare
and other services

Maintaining housing and landlord relationships

How Does The Program Work?

You will work one-to-one with staff to:

Set goals
Build skills for independent living
Connect with supports in the community

Support may begin before or shortly after you move into housing. Services continue based on your needs and goals.

Costs

There is no cost for support services provided through this program. Clients are responsible for their own living expenses including:

Rent
Food
Furnishings
Transportation and personal expenses

 

Staff can help clients explore funding options, if required.

How to Apply

Housing with Supports is usually accessed through referral. Referrals can be made by:

» Hospitals or transitional programs
» Community organizations
» The individual

An assessment is completed to determine eligibility.
