Responsibilities:

 The Community Access & Intake Specialist is responsible for providing the leadership and hands on coordination of centralized intake, assessment and referral of clients to transition to a reintegration care unit, supportive housing sites, or attendant outreach services.

  • Provide formal and informal marketing and training support for Bellwoods programs. Target audiences include hospital discharge teams, discharge coordinators and Social Workers
  • Develop, maintain and manage a centralized repository of program related information
  • Provide information about programs, services, and the admissions process to clients, families, and referral sources in a professional, client-centered, and compassionate manner
  • Maintain and process referral forms from Hospital discharge teams for intake into the Reintegration Care Units (RCUs), Supportive Housing Sites, and Attendant Outreach Services
  • Maintain communication and status updates from RCU providers as required to complete the intake process
  • Ensure the provision of timely and appropriate centralized intake services in a manner consistent with the policies and philosophy of the organization, and within established budgets and controls
  • Coordinate logistics for new admissions, including appointment scheduling, file preparation, and follow-up communication with clients and care teams
  • Provides a professional resource function for Bellwoods for related intake design, presentation or implementation strategies.
  • Lead for engagement with the Attendant Services Application Centre (ASAC)
  • Assist with initial intake and screening calls, gathering essential client information and determining basic eligibility or referral needs.
  • Supports the Service Excellence & Innovation (SEI) team to identify goals and objectives with clients which will enhance their level of independence in the community and overall quality of life, as well as completing and documenting an assessment of those aspects of a client’s situation which relate to identified goals and objectives.
  • Supports the Supervisors in designing and implementing an effective individual learning plan for clients, consistent with clients’ goals and objectives.
  • Records accurate and clear notes of meetings with clients and other actions on behalf of the clients
  • Assists in the planning, implementation and evaluation, in conjunction with the Director, Transitions and Community Partnerships, of new Program initiatives
  • Provides a potential resource and support to other program staff.

The Role:

The Procurement Specialist leads and supports procurement activities across the organization, ensuring compliance with the Broader Public Sector Accountability Act (BPSA). In addition to this, the procurement specialist is responsible for the supply and inventory management at Bellwoods. This role involves working closely with stakeholders to develop Requests for Proposals (RFPs) and Requests for Information (RFIs), facilitating the evaluation of proposals, supporting contract management and oversees the management of its frontline and administration-related inventories and supplies. The Procurement Specialist will play a key role in ensuring that all procurement and inventory and supply management processes are transparent, efficient, and aligned with organizational standards.

The Procurement Specialist reports directly to the Vice President, Finance/CFO, and ultimately, to the Chief Executive Officer.

Key Responsibilities:

  1. BPSA Compliance
  • Ensures that all procurement activities adhere to BPSA requirements, maintaining compliance and ethical standards across all processes.
  • Regularly reviews procurement practices to align with regulatory updates and organizational policies.
  1. RFP and RFI Preparation
  • Collaborates with stakeholders by leading the process of preparing RFPs, RFIs, and other procurement documents, ensuring clarity, accuracy, and completeness.
  • Provides guidance to departments on best practices for developing clear and effective procurement documentation.
  1. Proposal Evaluation Support
  • Works closely with business teams to ensure adherence to the evaluation process for RFPs and RFIs.
  • Assists in the assessment and scoring of proposals, ensuring a fair and objective selection process.
  • Ensures evaluation criteria are consistently applied and documented for transparency and accountability.
  1. Supply and Inventory Management
  • Ensures adequate office and personal protective equipment (PPE) supplies by reducing and/or avoiding significant excess or shortage of inventory.
  • Prepares report and analysis for various stakeholders on inventory usage and cost trends.
  • Identifies and implements cost effective and transparent processes to distribute supplies/inventories across sites that promotes transparency and accountability.
  1. Contract Development and Management

Required Skills and Qualifications:
